Innate’s Intake system provides a web-based platform for the most common intake and consent forms for new patients, updates, and outcome assessments.
Either when you schedule an appointment ahead of time or when you check-in a patient, Innate will generate a unique PIN (with the click of a button ) assigned to the patient for 1-time use. The patient enters this PIN and their DOB at the intake web link to access and complete the forms that you assigned them. When the forms are completed, the data is imported directly into the patient chart in written form, saving time and eliminating the chance of transcription errors.
